چکیده

Functional neuroimaging, such as positron emission tomography (PET)andfunctional magnetic resonance imaging (fMRI), provides a valuable technique for detecting regional changes in brain metabolic activity associated with human disease. These techniques have been applied in different dystonic disorders including primary generalized dystonia and dopa-responsive dystonia (DRD), as well as focal dystonic syndromes such as torticollis, writer’s cramp, and blepharospasm. A common finding is abnormality of thebasal ganglia and associated outflow pathways to sensorimotor cortex and other regions involved with motor performance. Other recent imaging research has utilized diffusion-based MRI techniques to localize distinct microstructural abnormalities in dystonia patients and gene carriers. This presentation will focus on an integrated approach to understanding the pathophysiology of this genetic and biochemically diverse disorder. J. Med. Invest. 52 Suppl. : 272-279, November, 2005

Genotype–phenotype interactions in primary dystonias revealed by differential changes in brain structure

Our understanding of how genotype determines phenotype in primary dystonia is limited. Familial young-onset primary dystonia is commonly due to the DYT1 gene mutation. A critical question, given the 30% penetrance of clinical symptoms in DYT1 mutation carriers, is why the same genotype leads to differential clinical expression and whether non-DYT1 adult-onset primary dystonia, with and without ...

The Dystonic Brain: Electrophysiological Investigation of Carriers of the DYT1 Gene Mutation

A mutation in the DYT1 gene on chromosome 9q34 is the commonest cause of young-onset primary dystonia. The penetrance of clinical symptoms is low (only 30-40% of gene carriers manifest dystonia), and occurs in an agedependent fashion. Mutation carriers who pass their mid-twenties without developing symptoms almost invariably stay symptom free for life. DYT1 mutation carriers therefore provide a...

Different patterns of electrophysiological deficits in manifesting and non-manifesting carriers of the DYT1 gene mutation.

A mutation in the DYT1 gene on chromosome 9q34 causes early-onset primary torsion dystonia with autosomal dominant inheritance but low phenotypic penetrance. The aim of the present study was to assess the functional consequences of the DYT1 gene, by comparing the electrophysiology of cortical and spinal circuits in clinically affected and unaffected carriers of the DYT1 gene mutation. We assess...

Impaired sequence learning in carriers of the DYT1 dystonia mutation.

Previous positron emission tomography (PET) studies have shown that nonmanifesting carriers of the DYT1 dystonia mutation express an abnormal pattern of resting glucose metabolism. To determine whether motor behavior is impaired in these subjects, we compared movement and sequence learning in 12 clinically unaffected DYT1 carriers with 12 age-matched controls. Regional differences in brain func...

Increased risk for recurrent major depression in DYT1 dystonia mutation carriers.

BACKGROUND Prior studies suggest that dystonia is comorbid with affective disorders. This comorbidity could be a reaction to a chronic debilitating disorder or expression of a predisposing gene. The authors took advantage of the identification of a gene for dystonia, DYT1, to test these alternative explanations. METHODS The authors administered a standardized psychiatric interview to members ...
